(SH-26) – Safety Management Self-Assesement – Deadline Approaching – 07/31/25

NOTE: When you completed your annual Progress Report in March, you may have been instructed to complete the Safety Management Self-Assessment (SH-26) at that time. If so, you have already completed this requirement and DO NOT have to complete it again.

If you do not remember completing it in March, you can log on (using the instructions below). If you completed it in March, you will receive a message saying it has already been completed. If you do not receive the “already been completed” message, complete and submit the form now.

This “Safety Management Self-Assessment” form is to be completed online, as follows:

**IMPORTANT: Upon completion of the form, PRINT a copy before you “submit” it!

1. Go to: bwc.ohio.gov
2. Log in to your E-Account. You will need to enter your company’s BWC user ID and Password, and click “log on”. (If you do not have a BWC User ID & Password, use the “How to create an e- account” link on this page to create one.)
3. Your “My Policy” page will appear
4. On the top of the screen Click on “INFO FOR EMPLOYERS”.
5. Click on “EMPLOYER FORMS” on left side menu
6. Click on “HERE” under – you’ll find a complete list of employer forms here
7. Scroll down to the form “SH-26 – Safety Management Self-Assessment”
8. Click the “Computer icon”.
9. The “Safety Management Self-Assessment (SH26)” page appears.
10. Click the “Start” button at the bottom of the page
11. On the next page, enter your BWC policy # and click “next”
12. You are now at the Safety Management Self Assessment form and can begin your on- line completion of the form.

Review your answers. If you need to change an answer, it will allow you to make changes BEFORE you “submit” the form.

REMINDER: Be sure to PRINT your completed form BEFORE you click “submit”!
